Pre-Participation Physicals

We will be having pre participation physicals Thursday, May 16, 2024 from 4:00pm-6:00pm in the APK Gym.

Physicals will be $20 cash only.

Please turn in your form to a Coach or Mrs. McGee as soon as possible.  Parents do not need to be present as long as the paperwork has been filled out and signed by a parent ahead of time. 

Students that are required to have a new physical are current 6th graders wanting to participate in athletics, current 8th graders, and current 10th graders who will be in athletics, cheer, or band.

Lexington ISD Athletic Physicals

 4-6pm in the High School Gym

                                  

The Lexington ISD Athletic Department in conjunction with the Texas A&M Residency program have designated the afternoon of May 16th, 2024 from 4pm-6pm for preparticipation physical examinations for our students.  A physical is a requirement for those students desiring to participate in extracurricular activities for the 2024-2025 school year. (those entering 7th, 9th, and 11th grade)


***Students must have physical and all required online forms on file before they will be allowed to participate in practice or games of their respective sport(s) and/or marching band/ or cheer. ***


 




  • Rankone online athletic participation forms instructions.

  • Lexington ISD uses an online database system for participation forms called RankOne Sport. 
  • The second page in the packet has all of the information you will need to help you correctly fill out the forms online. 
  • There is no need to print out any extra forms, once you have completed the forms online they will automatically get submitted to the school. (All boxes must be filled out and all signatures must be submitted for the forms to be completely accepted)

The forms can be filled out on computers as well as most smart phones.



  • Pre-participation Medical History/ Physical Evaluation
    • The first page is the student's medical history questionnaire.
    • Please complete the entire 1st page along with parent/guardian and students signature at bottom. If there are any yes answers please provide an explanation in the box at the bottom.
    • On the back of the 1st page is where the doctor will fill out the physical exam findings.
    • Please fill out only the first line on 2nd page (name, sex, age, DOB)

You will be able to submit all athletic paperwork on this website. 

  •  From this link you can download and print a physical form to take to the doctor if you need a physical .  
  • The completed physical form needs to be uploaded into the rankone portal or turned in to a coach, band director, or athletic trainer. 
  • Click on the PROCEED TO ONLINE FORMS button and from there you can either create an account or continue as a guest to the online forms. 

The last name used should be the last name used to register your student with the district, as well as their student ID number. 

The forms you will be asked to fill out online are:

 - Drug Testing Consent

-Medical History

 -Student permission form 

-UIL form Signature page

-Physical Upload

-Emergency Card

*Every section of each form needs to be completed for the form to be submitted.*
